Follow up inspection to the routine inspection conducted on 07/23/2025 during which facility was subject to closure due to an active cockroach infestation. Observations : No signs of active infestation observed. Pest control documentation and checklist has been received and reviewed. Sanitation and food safety measures have been verified as completed. Prevention measures on the checklist have been verified as completed. Scheduled for ongoing pest service documentation provided. Facility os okay to operate.

ROUTINE INSPECTION

CO0157389 Limited inspection conducted in conjunction with a complaint investigation. Live cockroach activity was observed throughout the kitchen, near food preparation areas and in the front area. Facility is closed due to the active vermin infestation. Facility shall remain closed until the vermin issue is resolved and a re-inspection confirms the facility is okay to operate. A vermin checklist has been provided to the manager. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to : kaya.alassfar@deh.sccgov.org. 2. Submit a copy of the pest control report from a licensed provider.

Notification: The person in charge during inspection, Uri Calderon, has been informed that the facility must close immediately.
