Notes: - A risk factor Assessment/Structural Review was provided during non-operation hours. Provided inspection, discussion and education focused on observed and non-observed risk factors know to contribute to foodborne illness. - Facility only operates their kitchen 3-4 times per year. The hall, including the kitchen and bar, are rented for private events throughout the year. - Facility is currently permanently exempt from permit fees. This classification will be changed to reflect current department policies. Facility contact information and billing was verified with the person in charge onsite. - Discussed handwashing, warewashing, cooking, hot and cold holding, sanitation, employee habits, and employee health with PIC.

1. The customer restroom handwash station lacked soap. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. [COS] PIC provided a pump bottle of soap to the handwash station. 2. The handwash station at the bar lacked paper towels in the dispenser. A stack of paper napkins were provided as an alternative. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels.

1. The Environmental Health permit was not posted in public view. [CA] The health permit issued by this department shall be posted in a conspicuous place in the food facility. 2. Lavatories were missing handwash reminder signs. [CA] A sign or poster that notifies food employees to wash their hands shall be posted at all handwashing stations used by food employees.

Facility lacks testing materials for their choice of sanitizer at the kitchen 3 compartment sink and upright dishwasher, chlorine. Note: Facility had testing materials for their quaternary ammonia based tablets at the three compartment sinks in the bar. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.
