    **REPEAT VIOLATION** 1. Observed vegetables stored outside in a makeshift storage tent. The storage tent is not fully enclosed and has holes/tears throughout. Per PIC, foods were recently delivered... [CA] Discontinue storage of food in the tent. All food must be stored inside fully enclosed and approved food facility. 2. Observed food stored on the floor throughout the facility: in the walk-in cooler and freezer, storage areas, and throughout the display shelves. [CA] Food shall be stored at least six inches above the floor to prevent contamination and pest harborage.

    Observed several flies throughout the facility. [CA] The premises of a food facility shall be kept free of vermin and other non-disease carrying insects, fruit flies, ants, weevils, and gnats.

    Observed several prepackaged foods for sale by "Tawaplatter" missing labeling information, including nylon poha chivda, multigrain cookies, thelpas, ravastick, fafda, nankhatai, etc. Per owner over the phone, foods are prepared at a commercial kitchen. No Environmental Health Permit or Processed Food Registration for this operation was available for review. [CA] All food shall be obtained from an approved source. Food prepared in an unapproved kitchen or private home may not be used or offered for sale in a food facility. [COS] All food was removed from the sales floor at the time of inspection and will be picked up by the vendor.

    Observed air curtain above the back entrance was not in operation. The back entrance door was kept open for active receiving during inspection. [CA] Air curtains shall be in operation over all open exterior doors to prevent flying pests.
