FOLLOW-UP INSPECTION

The facility was closed yesterday because of cockroach infestation. The corporation also removed all the equipment, the panel, and conducted a thorough cleaning behind the cook line. A pest control company came and conducted an intervention as well. During the time of this inspection, no live or dead cockroach was observed. Therefore, the facility is open for service. However, a regular thorough cleaning and pest control intervention must be conducted to prevent any future infestation.

Note: This limited inspection is conducted for an insect complained to reported to our office on 11/06/24. During the time of this inspection, several live and dead cockroaches were observed on the floor and wall of the cook line. Therefore, the facility shall be closed until all the live and dead cockroaches are eliminated from the facility.

[CA] Therefore, the facility is closed until a pest control company conducted an intervention, eliminate any insects from the facility, and sanitize the food contact area after. After hour or weekend follow-up inspection will only be conducted at a charge of $645.00/hour re-inspection fee. Suzanne Lew, Supervising Registered Environmental Health Specialist approved the closure.
