    Rodent droppings observed scattered under shelves along walls of facility and in back equipment area. Rodent droppings also observed under the end caps of aisles. One dead adult cockroach observed in trap behind screen where chairs and tables are stored. [CA]Facility shall clean and sanitize all droppings and dead cockroaches from affected areas. Ensure traps changed regularly. Follow up shall be conducted to ensure droppings have been cleaned. Facility shall provide last pest control report and contract with pest control during follow up inspection.

Owner requested follow up inspection. Observed several rodent droppings that appeared to be old. Under two shelves. No other evidence observed. Facility has cleaned and sanitized throughout, removed shelving units and cleaned underneath. Pest control report reviewed. Facility ok to opened.

ROUTINE INSPECTION

Facility is hereby closed due to evidence of a vermin infestation. Facility is to remain closed until: - Facility has been serviced by licensed pest control company. Provide pest control report to review upon follow-up inspection. - Remove all rodent droppings. - Clean and sanitize all areas affected. - Facility is to remain closed until authorization is given by this Division to reopen. - Contact district inspector once major violation has been corrected and checklist items have been completed. Jennifer Rios 408-918-3480 - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. - A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors.     Observed rodent droppings around food containers on pallets observed rodent droppings on the floors by shelves. Observed two dead rodents on traps. Observed dead cockroahces on glue traps. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
