Observed food equipment and utensils stored on rolling rack inside the restroom.
[CA] All cleaned utensils shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least six inches above the floor.

Heavy accumulation of syrups and food coloring observed on walls at the cake decorating area.
[CA] Walls and/or floors in food preparation area shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.
Numerous missing ceiling panels and/or holes on ceiling panels throughout the kitchen.
[CA] Ceilings of all rooms where nonprepackaged food is handled shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable.

[CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

Hand wash station at the ware washing area completely obstructed with baking trays and empty egg flats stored at the basin.
Hand wash station in the restroom obstructed with rolling racks of food equipment/utensils, and mop bucket.
Paper towels at both hand wash stations observed discolored and appear to have not been used for a period of time.

Observed facility to be heavily cluttered with equipment. Three-door reach-in refrigerator where vermin infestation was observed is inoperable and is used as a preparation surface. =
[CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.
Mop stored inside mop bucket that is maintained inside the restroom.
[CA] After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Observed employee eating lunch at the cake decoration area.
[CA] No employee shall eat or drink in the food prep, food contact storage, or food storage areas, use designated area.

Observed 6+ live and dead cockroaches with numerous carrying egg sacs observed at the inoperable black three-door reach-in refrigerator at the cake decorating room, between the top of the doors and underside of the surface of the refrigerator. Numerous dead cockroaches observed inside the refrigerator as well.
5+ live cockroaches observed inside the corner cabinet at the front counter, under the bread slicing machine. Cockroaches were observed crawling on top of bags and boxes used to store food.
Per PIC, facility is serviced monthly, with last service conducted on 8/17/2024. Pest control reports were unavailable for review.
[CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
Numerous fruit and/or drain flies observed throughout the facility.
