Follow- up inspection to the limited inspection conducted on 4/15/2025. Food facility was closed due to an active vermin infestation and instructed to cease operation of food sales. Observations: -No evidence of a vermin infestation observed in any aisle of sales floor. -No evidence of a vermin infestation observed in the stock room. -Holes in ceiling above oral care (aisle 26) and hair care (aisle 2) have been repaired. Note: Checklist for reopening/pest control reports were submitted via email on 6/24/2025 by store manager. (Nadine El-Saleh) Facility is now OPEN.

    Prepackaged food items stored in outdoor unapproved portable storage unit. Per manager, food items were removed from facility during active vermin infestation. Portable storage unit was not previously approved by this department. [CA] Food shall be protected from contamination by storing the food in an approved, clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Note: Store manager stated that all food items in outdoor storage unit will be voluntarily discarded. Manager placed work order for a dumpster rental with SCPM during inspection.

ROUTINE INSPECTION

Facility is hereby closed due to active vermin infestation. Facility shall cease operation of food sales immediately. Note: During inspection, manager blocked off areas of food sales and placed signs stating "No food sales until further notice." Pest control reports from 4/7/25, 4/4/25, and 4/3/25 were provided by store manager. Note: Store manager stated that he found a bag of pistachios with gnaw marks on it this morning. Limited inspection conducted in conjunction with complaint investigation: CO0156637 This is a chargeable inspection: $298/hour, minimum one hour. Subsequent follow-up inspections will be billed $298/hour, minimum one hour, during normal business hours. (Monday-Friday, 7:30am to 4:30pm) Please contact district specialist, Anabelle Garcia at (669)946-4685 or DEH main line at (408)918-3400 when areas of old rodent droppings have been thoroughly cleaned and sanitized. Follow up inspection to the routine inspection conducted on 03/25/2025. Facility ceased operation of food sales due to food adulteration and rodent droppings found throughout facility. K13: All packaged foods observed in good condition, no rodent gnaw marks. K23: No rodent droppings observed on floors or shelves of food sale areas. No urine odor present. All affected areas have been cleaned and sanitized. Food facility is now OPEN.
