A follow up is requested today by the owner for closure on 12/02/2025. 1. Observed Vermin: Documented in the following areas: Observed 1 live cockroaches on top of the flour bin. Multiple dead cockroaches observed in the following locations: - 1 behind soy sauce bucket storage - 3 on floor next to the employee lockers - 2 on front counter near plant - 2 on top of flour bin in the ware house - approx. 5 in front of the warehouse entrance - 6 behind the dough mixer - 4 in kitchen - 1 on electrical outlet in the eggroll room Observed last pest control service report dated, 12/02/2025, stating no activity observed. 2. Photographs: Taken for documentation purposes: yes 3. Supervisor Notified: yes 4. Notification: The person in charge during inspection, Paulette Davis, has been informed that the facility must remain closed. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector.
