FOLLOW-UP INSPECTION
Follow up inspection for routine inspection on 02/27/2025 regarding evidence of dead cockroaches. During the inspection, Specialist found no evidence of cockroaches at the facility. Per Manager, Terminex came today to conduct a pest control service. Service report provided and noted German cockroach activity. Manager stated that the facility shall now receive monthly pest control services from Terminex. Ensure continued monitor activity for vermin and to follow pest control company recommendations. Maintain food and non-food contact surfaces clean and free of debris. Discussed ensuring all entry doors are maintained closed to prevent entrance of vermin.
ROUTINE INSPECTION
OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: RAW SUPERFOOD CAFE NEW OWNER: 39 RAW SAN JOSE, LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP 10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. A 25% penalty fee shall also apply in addition to the $1,107.00 permit fee for operating without a valid health permit as stated on routine inspection conducted on 02/17/2025. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 03/01/2025 - 02/28/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 02/27/2025 Notes: PIC stated plans on assessing current hood system because it is not sufficient for its operation. Discussed with PIC that any new construction, remodeling or structural change to the food facility, removing or installing new equipment, or any significant menu change, must submit complete plans to DEH Plan Check. Minor yellow staining on the wall underneath Type 2 hood. [CA]: Maintain walls clean. Note: if type 2 hood is found not sufficient for current food operations, facility may be required to reach out to DEH Plan Check for hood replacement/assessment or to scale down food operation.
Multiple dead cockroaches of varying life stages found in the following areas: -on floors underneath the prep sink and 2 door under counter cooler -on floors behind 2 door reach in freezer -on floors near mop sink Per PIC and manager, residential pest control repellent is used to abate/treat cockroaches. No live cockroaches observed after thorough inspection of the facility. [CA]: Eliminate evidence of cockroach in a safe, effective and legal manner. Ensure food and food contact surfaces are protected from possible contamination at all times. Specialist recommends following professional pest control service recommendations and monitor for vermin activity. Seal all cracks, crevices, and gaps under any exterior doors to prevent entry and harborage of vermin.

Open bags of dry food at the dry storage area. [CA]: All open bags of dry food shall be transferred or stored in food grade containers and covered with a fitted lid.
Residential pest control repellent observed for the treatment/abatement of cockroaches. [CA]: Commercial establishments are not approved to use household/residential pest control methods. Discontinue storage/use of unapproved pest repellents.

FOLLOW UP INSPECTION FROM 2/17/25 IN WHICH THE WALK IN COOLER WAS UNABLE TO KEEP THE POTENTIALLY HAZARDOUS FOOD AT 41F OR BELOW. FINDINGS: THE WALK IN COOLER WAS ADJUSTED AND IT IS NOW ABLE TO KEEP THE POTENTIALLY HAZARDOUS FOOD BELOW 41F.

THERE HAS BEEN A CHANGE OF OWNERSHIP IN 8/2024 FOR THIS FACILITY, PER OPERATOR. THIS A 48 HOUR NOTICE TO SUBMIT A CHANGE OF OWNERSHIP AND FEE EVALUATION APPLICATION AND FEE. FOUND THE PLANT BASED SPREAD AT 52F AND THE SLICED TOMATOES AT 49F STORED IN THE SERVICE REFRIGERATOR. [CA] STORE POTENTIALLY HAZARDOUS FOOD AT 41F OR BELOW. NOTE: THE TEMPERATURE SETTING FOR THE REFRIGERATOR WAS SET HIGH, THE OPERATOR TURNED IT DOWN TO 37F. FOUND SEVERAL SAUCES AND PLANT BASED MEAT AT 46F STORED IN THE WALK IN COOLER FOR OVER 4 HOURS. [CA] COLD POTENTIALLY HAZARDOUS FOOD SHALL BE STORED AT 41F OR BELOW. SEE VC&D.

FOUND A SAC OF ONIONS STORED ON THE FLOOR IN THE WALK IN COOLER AND SOME BOXES WITH FRUIT ON THE FLOOR IN THE WALK IN FREEZER. [CA] STORE FOOD 6 INCHES ABOVE THE FLOOR.
THIS FACILITY IS OPERATING WITHOUT A PERMIT. [CA] APPLY FOR A PERMIT WITHIN 48 HRS TO AVOID CLOSURE.
