This was a follow-up to the 10/9/25 (routine/closure) and 10/14/25 (first follow-up) inspections. No cockroaches, dead or alive, were observed in the facility. Facility is OK to re-open. Discussed other cited major violations (handwashing, holding temperatures) with operator. No PHFs were stored inside walk-in refrigerator (only raw vegetables); vegetables measured 44F. Operator stated they will discard the vegetables since it has been several day since they were closed. Do not store potentially hazardous foods inside the walk-in refrigerator until it can keep foods at 41F and below. Continue cleaning areas with grease accumulation (e.g. in prep unit in front cooking area).

This is a follow up inspection to the routine inspection conducted on 10/09/2025 requested by management. Received/reviewed pest control reports and signed checklist. Continue to eliminate harborage areas for the cockroaches and continue to clean evidence of cockroaches. Ensure that foods are properly cooled prior to placing them into cold-hold inserts, as the units are not designed to cool down food, but only to maintain at 41F or below. Note: This is a follow-up inspection to the limited inspection conducted yesterday by Lawrence Dodson. The facility was closed due to cockroach infestation. During this follow-up inspection, Multiple live and dead cockroaches were observed back in the dry storage areas and some under the front handwash station. Therefore, the facility shall be remain closed. The owner shall be billed $645.00 for this after-hour follow-up inspection.
