- On-site for follow-up inspection. - K23m - Observed rodents, insects, birds, or animals: - Facility has deep cleaned and sanitized all affected areas. - No rodent droppings were observed throughout the facility. - Pest control serviced facility on 5/27/2025 with no pest activity found. - Pest control report was provided for review. - K42m - Garbage and refuse improperly disposed: - Refuse areas observed organized with minor accumulation of garbage on floor next to the compactor. - Manager stated that refuse area is cleaned nightly and the roll-up doors are only opened when the trash bin is replaced. - Manager also stated that other restaurant facilities that utilize the compactor usually leave garbage littered around the compacter. - Continue to clean and maintain facility free of vermin.

[CA] Ceilings of all rooms where nonprepackaged food is handled shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable. Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.

    Old rodent droppings observed in the following areas: - Dry storage room: - On floor underneath alcohol shelves and chemical storage shelves. - On top of bucket of chemical. - Two on top of ice machine outside of the dry storage room. Per manager, facility is serviced monthly, with the most recent service conducted on 5/23/2025 in the morning. Pest control report was provided for review. Per report, "no rodent or insect activity was noted during the inspection and/or service." [CA] Clean and sanitize area of old droppings.

MANAGER EUNICE LOPEZ WAS INFORMED ABOUT THE FOLLOWING COMPLAINT: 1. ANONYMOUS COMPLAINT RECEIVED ON 7/31/24 STATING FILLET MIGNON HAD AN EXPIRED SMELL INCLUDING SIRLOIN & LAMB 2. ANONYMOUS COMPLAINT RECEIVED ON 10/9/24 STATING STEAK WASN'T GOOD / OLD, SUSPECTS RESTAURANT MAY BE REHEATING. CARPET IN DINING AREA HAD MOLD AND WAS ODOROUS.
