    Couple of drain flies were observed by the janitorial sink area. [CA] Eliminate all insects and rodents from the facility by a legal method.

[CA] To prevent insect hiding and breading and for easy cleaning of the floor area, please store all food products at least six inches off the floor on approved shelf or dunnage rack.

    The faucet of the handwash station is leaking. [CA] Repair the plumbing structure immediately. Drain from the walk-in cooler is directed into the restroom's handwash station. [CA] Drain from condemnation of refrigeration unit or equipment must be directed into a floor sink. The drain must be fixed in such a way that it shall drain into a floors. Modifying plumbing structure requires plan submission and approval from our plan check unit and the city's building department.

The PIC removed the samosa from the hot-holding case and voluntarily condemned and destroyed them (COS).
