    Observed old rodent droppings on the floors behind/below the stove. [CA] Clean and sanitize area of old droppings.

    Observed gaps around the conduit in the corner cabinetry to the right of the stove. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.

    Observed raw shelled eggs stored above ready to eat foods in the walk in cooler. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

    Testing equipment was unavailable for the high temperature dish machine. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

    Observed the lights in the walk in cooler to be unshielded. [CA] Light bulbs shall be shielded, coated or otherwise shatter-resistant in areas where there is unpackaged foods, clean equipment, utensils, linens, or unwrapped single use articles.

FOLLOW-UP INSPECTION

On-site to conduct a follow-up to the routine inspection from 6/16/2025. The following major violations have been corrected: - K06. Adequate handwash facilities supplied. The handwash sink in the kitchen is fully stocked with soap and paper towels. Ensure handwash sink is fully stocked and accessible at all times. - K07. Proper hot and cold holding temperatures. The walk-in cooler has been repaired and is now maintaining all foods at 41F. See measured observations. - K13. Food in good condition and unadulterated. Observed no live activity of ants inside the kitchen and no direct contamination in the food. Per PIC, facility set up Terro "Ant Killer Liquid Ant Baits" bait stations around the premises. Ensure that bait stations are not used within the kitchen. Facility is advised to seek routine professional pest control services.

ROUTINE INSPECTION

A follow-up inspection will be conducted to verify compliance of major violations. The first follow-up will be free of charge. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour during non-business hours, and upon inspector availability. This report was written and signed by the PIC at the end of site inspections.

    Observed active ant infestation inside the pull-out cabinet for food storage. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies. *NOTE* Domestic pesticides or chemicals are not approved for use in a food facility. Recommend to seek professional pest control services.

    1. Observed numerous trails of ants inside the pull-out cabinet for food storage. Ants were found centered on a container of honey and bunch of bananas. [CA] Food shall be maintained free of adulteration. [COS] Container of honey and bananas were discarded. See VC&D report. All other open food containers inside the pull-out cabinet were cleaned and sanitized of ants. 2. Observed dead weevil-like species on top of sugar inside a plastic container. [CA] Food shall be maintained free of adulteration. [COS] Sugar was discarded. See VC&D report. Plastic container to be cleaned and sanitized.

    Observed soap dispenser at the only handwash sink in the kitchen is empty. [CA] Handwash sink shall be supplied with soap and single use towels at all times. [COS] PIC stocked handwash sink with portable bottle of soap.

    Measured the ambient temperature of the walk-in cooler at 52F. [CA] Refrigeration unit shall maintain foods at 41F or below. Discontinue the storage of Potentially Hazardous Foods until refrigeration unit is repaired.

    Measured multiple Potentially Hazardous Foods in the walk-in cooler from 50-53F, including cooked pork, deli corned beef, turkey patties, sausages, and cheese. Per PIC, foods have been stored in the walk-in cooler overnight. [CA] Potentially Hazardous Foods shall be kept at 41F or below or 135F or above. [COS] All Potentially Hazardous Foods were voluntarily condemned and discarded. See VC&D report. Raw meat that was just delivered was relocated to upright refrigeration units for cooling.
