Observed large opening in the basement behind the water heater. [corrective action] Patch the opening.

1. Hot water at the 3-compartment sink measured 111F. [corrective action] Provide hot water, minimum 120F. 2. Water from both sinks in the women's restroom measured 66F to 68F. [corrective action] Provide warm water, minimum 100F.

1. Men's restroom stall was lacking a door. [corrective action] Replace the door. 2. Toilet in the men's restroom appeared to be in bad condition. Tape was used at the base of the toilet. [corrective action] Ensure toilet is in good repair. Replace the toilet.

Hand sink at the bar was lacking a paper towel dispenser. [corrective action] Provide a paper towel dispenser at the bar hand sink and stock with paper towels. [suitable alternative] A stack of napkins were provided at the bar.

1. Observed leak from faucet at the 3-compartment sink when both hot and cold water levers were off. [corrective action] Repair the leak. 2. Observed water leaking under the bar hand sink. [corrective action] Repair the leak.

Repeat Violation: Hand sink at the bar lacking a paper towel dispenser. [CA] Install a paper towel dispenser at the bar.

Repeat Violation: Large opening in the wall behind the water heater in the basement. [CA] Maintain the floors, walls, and ceilings of the facility. Patch the wall to prevent entrance and harborage of vermin.

The middle ice bin has tubes for the soda gun running through ice meant for consumption. [CA] Ice intended to be used for food or beverages shall not come in direct contact with materials that may cause contamination.
