OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: PIZZA BHAVAN NEW OWNER: QUANTUM FOOD GROUP LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $789.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 5/01/25 - 04/30/26 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 4/4/25. *Obtain food safety manager certificate within 60 days. Note: This limited/routine inspection is conducted for a change of ownership for SR0880304 NEW OWNER: Helma LLC NEW FACILITY NAME: The Sandwich Spot The applicant has completed the application for an Environmental Health Permit. FP10 $1,107.00 will be invoiced and mail to the address on the application after our plan check unit approved the water heater installation. Payment shall be paid within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received within 30 days of the change of owner date. Permit issued and valid: The permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. This report serves as a temporary permit until the official permit is mailed to the facility. When the invoice for the facility is received, payment must be done within 10 days. Okay to operate as of today. The official Environmental Health permit must post permit in public view and copy of the most recent inspection report must be kept on file for review by anyone upon request. Someone from the facility must take a food manager certificate class within two months. All employees who are engaged in food preparation service, and storage, and who do not have a food manager certificate must get a food handler card within a month of hire. *Structural Review inspection conducted on 4/4/25. Since the water temperature did not reach 120 oF on the three compartment sink, the restaurant shall be closed. A permit is not issued to the new owner. Please submit specification of water heater tank to our office for evaluation before installing it.
