[CA] Food facility shall not operate if there is sewage overflowing or backing up in the food facility. The food facility, or impacted areas, shall remain closed until all plumbing problems have been corrected and all contaminated surfaces have been cleaned and sanitized and a follow up inspection is conducted by this department.

FOLLOW-UP INSPECTION

Follow-up for facility closed due to vermin infestation. Pest control service report and checklist for reopening was received and reviewed. Facility obtained pest control service on 12/23/25 and an additional service on 12/24/25. Service report from 12/24 notes that there no activity found and follow-up services will occur within the next two week. All areas where droppings and cockroaches were previously observed has been cleaned and sanitized. No live or dead vermin was observed in the facility at time of inspection, and there were no evidence of an active infestation. Holes and gaps in the facility that were noted during the previous inspection have been patched and sealed. Facility is okay to reopen.

    1. Observed Vermin: Documented in the following areas: 1 live cockroach observed in the cabinet by the hand wash sink in the front preparation area. Observed one live cockroach under the three compartment sink and one live cockroach by the janitorial sink in the back preparation area. Observed one dead cockroach in the cabinet in the coffee bar. Observed rodent droppings in the front service area in the cabinets, in the cabinets in the satellite service area. Observed many rodent droppings around the packaged chips and a chewed on cheese back in the upstairs storage area. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: S. Lew 4. Notification: The person in charge during inspection, Jasmin, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.
