On-site for a follow-up inspection after facility was closed due to evidence of a cockroach infestation on 10/02/2025 The following was provided prior to the follow-up inspection: 1) Pest control report from a licensed provider stating that the facility was treated and that no vermin activity was observed after treatment 2) Signed and completed Vermin Reopening Checklist No evidence of cockroaches observed. Several areas still require cleaning, including the walls to the either side of the cook-line, the floor under some pieces of equipment, and on the sides and undersides of equipment/metal counter units. Facility has not removed all of the unapproved, "jury rigged" wooden material attached to counter units. As cited on the last routine inspection, facility is to remove said material, as it is not smooth, easily cleanable, durable, or nonabsorbent, and may provide harborage for vermin. Handwash sinks observed to be unobstructed and properly stocked with soap and paper towels. Facility may now be reopened. Continue to work on the violations cited during the last routine inspection.

ROUTINE INSPECTION

Facility is hereby closed owing to evidence of a cockroach infestation. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale. The following must be provided prior to reopening: 1. The signed and completed Reopening Checklist, emailed to the assigned inspector 2. A copy of the pest control report from a licensed provider, stating that facility has been treated and that all vermin activity has been abated, emailed to the assigned inspector Facility must also perform the following actions prior to reopening: - Eliminate all presence of vermin, dead or alive, within the facility. - Clean and sanitize all areas, including but not limited to floors, walls, ceilings, and surfaces of all equipment throughout the facility. - Seal any gaps, cracks, or holes within the premises and in the structure of the facility that may provide harborage or entrance for vermin. - Dispose of any contaminated items. - Remove unused equipment so as to prevent harborage for vermin. A follow-up inspection must be conducted prior to facility reopening. Call or email to schedule. Subsequent follow-up inspections after the first shall be billed at $298/hour, one hour minimum, during normal business hours (7:30 AM to 4:30 PM), or $645 for a minimum of 2 hours, during non-business hours and in accordance with the inspector's availability. Observed Vermin: Live and dead cockroach activity observed in the following areas: On the floor around the swinging doors to the entrance of the kitchen, on the floor at the front service area, on the floor under the handwash sink in the kitchen, on the wall to the right of the cook-line, on the wall separating the cook-line area from the prep area, on the floor in the left-side storage area, and on the wall to the left of the cook-line. 2.     *REPEAT VIOLATION* 1. Raw lamb measured 66F in a plastic tub on a counter in the prep area. Per staff, lamb had been taken out 3.5 hours prior. 2. Various potentially hazardous foods (chicken, beef, tofu, noodles, vegetable chowmein, etc.) in the steam table at the front service area measured in the range of 113F-131F. Per the PIC, they had been cooked 1 hour prior, or 2 hours prior depending on the item. 3. Chive and egg buns measured 94F while maintained out of temperature control on the counter at the front service area. 4. Raw pork measured 50F while in a box on the counter at the prep area. Per staff, pork had been received one hour prior. 5. Pork buns measured 75F while maintained inside plastic takeout boxes on the counter of the front service area. [CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan.
