  • K06: Adequate handwash facilities supplied, accessible

    Hand wash sink in the back area is difficult to access due to storage racks placed in the tight corridor. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. Relocate storage shelf so that hand wash sink is easily accessible.

  • K15: Food obtained from approved sourceCritical

    Facility is located inside shopping mall. This Division walked past facility was observed various sticky rice rolls and pyramid tapioca rice dumplings. When this Division's return to being inspection, employee stated that all items were sold to customer. Upon further investigation, this Division found approximately 40+ rolls (mix of sweet rice with pork (banh tet thit) and sweet rice with banana (banh tet chuoi), 10 pyramid tapioca rice dumplings with salted egg (banh u trung muoi), approximately 17 boxes of sponge cakes (banh bong lan), hidden and taped inside a box. When questioned about source of the items, manager stated she does not know, nor does she know the supplier. [CA] Food must be from an approved source or properly documented. [COS] All items were VC&D. See secondary VC&D report.

    Observed numerous prepackaged items offered for customer self-service lacked labels. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.
