The facility is only approved to sell prepackaged food items only. The cakes and bread in display refrigerator shall be be prepackaged. The facility receives all the food items from Milldang Bakery located at 1488 Halford Av, Santa Clara. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: MILLDANG BAKERY NEW OWNER: LORD IREH, INC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP76. An invoice for the permit fee in the amount of $268.00 + $67 = $335 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 5/1/2025 - 4/30/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 4/9/2025. *Permit condition: Facility lacks a hand wash sink and is only approved to sell prepackaged food only. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.

Lack of thermometer in display refrigerator. [CA] A thermometer +/- 2°F shall be provided and correctly located in each hot and cold holding unit containing potentially hazardous foods.

[CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

[CA] A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling. Contact DEH at 408-918-3400 or at www.ehinfo.org to obtain plan check requirements. Provide plans for new refrigerator no later than 4/25/25.
