[COS] Employee added more ice to the paneer to facilitate rapid cooling. 3) On the preparation counter, measured raw chicken at 52F. Per employee, raw chicken was removed from refrigeration less than 30 minutes ago (prior to cooking). [CA] Potentially Hazardous Foods shall be kept at 41F or below or 135F or above. [SA] Raw chicken was relocated to the upright cooler to facilitate rapid cooling.

    Observed improper handwashing practices by employees: - Observed employee skewering raw chicken with gloved hands, then skewering paneer with the same gloves without an intermediate handwashing step. - Observed employee handling contaminated and clean dishes from the dishwasher with the same gloves without an intermediate handwashing step. [CA] Employees are required to wash their hands: 1) before beginning work, 2) before handling food or clean equipment, 3) as often as necessary during food preparation to remove soil and contamination, 4) when switching from raw to ready to eat foods, 5) after touching body parts, 6) after coughing, sneezing, eating, or drinking, 7) after using the restroom, and 8) any time when contamination may occur. Gloves shall be changed with an intermediate handwashing step as required. [COS] Specialist intervened and instructed employee to properly wash hands and change gloves. The contaminated food was cooked for service and contaminated dishes were re-washed.

ROUTINE INSPECTION

On-site to conduct a commissary evaluation for: HYDERABADI KITCHEN and GUNTUR TIFFINS. At the time of inspection, observed active wastewater backup onto the floor from the 3-compartment sink whenever it is turned on. Facility is hereby CLOSED by this department due to failure to properly dispose of wastewater and lack of facilities to wash, rinse, and sanitize equipment. Facility is ordered to cease operations immediately until major violations have been corrected. - The first follow-up inspection is free of charge. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of 2 hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. - A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. Confirmed that facility is able to provide the following services for MFF operations: - Facilities to prepare foods. - Food storage: Per PIC, most food will be stored in trucks. - Equipment and utensils storage: Per PIC, no equipment and utensil storage on-site. - Potable water supply. - Supply food products. - Chemical storage. - Overnight parking: Per PIC, Guntur Tiffins will be parked at 3595 Benton Street, Santa Clara. Hyderabadi Kitchen will be parked at another location, under negotiation. - Garbage/grease/oil disposal. At the time of inspection, facility is unable to provide the following services: - Warewashing operations: See violations K14 and K22. Facility will NOT be approved for use as a commissary until the warewashing operations have been repaired. Contact district specialist for re-opening.

On-site to conduct a follow-up inspection to the facility closure (dated 10/15/2025). The following major violations have been corrected: - K14: Food contact surfaces clean and sanitized. Per PIC, technician arrived and repaired mechanical dishwasher. Retested chlorine sanitizer solution at 50 ppm during warewashing. - K22: Sewage and wastewater properly disposed. Observed liquid waste from the 3-compartment sink is now properly draining into drainpipes and does not overflow onto the floor. The gaps in drainpipe have been sealed with sealant. Facility is hereby authorized OPEN by this department. At the time of inspection, verified that facility is able to provide the following services for MFF operations: - Warewashing operations: 3-compartment sink and mechanical dishwasher. - See limited inspection report (10/15/2025) for other provided services. Facility is approved for use as a commissary.
