The inside of the walk in has accumulation on the walls and ceiling. [CA] Thoroughly clean the inside of the walk in cooler.

The water at the hand sink did not reach 100 degrees F and the dish sink did not reach 120 degrees F.[CA] The dish sink must reach 120 and the hand sink 100 degrees F. [COS] The manager turned up the hot water heater temperature and then reached proper temperatures when checked later.

Observed the drain pipes in the floor sink near the ice machine. [CA] Provide an air gap between the pipe and floor sink.

The green cutting board is deteriorated with plastic pieces coming off of it. [CA] Replace the deteriorated cutting board.

The floor in the walk in has grout missing and standing water in between the tiles. [CA] Repair the tile floors by adding the grout everywhere needed to help keep the water and debris from staying in between the tiles. Also repair the missing/broken tiles around the walk in cooler outside edges.

--48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $ 1435.00. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.
