    Sanitizer within the 3-compartment sink was measured at 400PPM at the time of inspection. No active ware-washing. [CA] Maintain chemical solution for manual warewashing at 100 ppm (1Tbs of bleach (5.25%)/gal of water) for chlorine and 200 ppm (1 tbs of quaternary ammonium (10%)/2 gal of water) for quaternary ammonium. [COS] PIC added water to the 3-compartment sink; re-measured at 200PPM.

    Observed employee touch personal phone and go to food prep. [CA] Properly wash hands with soap, warm water and dry using single use paper towels as required.

    Soap dispenser within the restroom is not functioning at the time of inspection. Batteries seem to be low. [CA] Provide hand washing cleanser in dispenser at hand wash stations at all times.

    Sanitizer buckets within the kitchen area were measured at 100PPM of quats at the time of inspection. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium). [COS] PIC changed out the sanitizer bucket; re-measured at 200PPM of quats.

    Under counter prep table next to the cook line was measured at 60*F at the time of inspection. PHFs within the unit were recently placed in there. [CA] PHFs shall be held at 41°F or below or at 135°F or above. Have the unit serviced by a contractor. [SA] PIC moved all the recently placed PHFs from that unit and moved them to the walk in.

    One of the restroom doors has a damaged door handle at the time of inspection. [CA] Toilet facilities shall be maintained clean and in good repair. [SA] Facility has an additional restroom that could be used.

    Mold like substance is growing within the caulking around the 3-compartment sink. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
