Your balance due is $557.50 Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A copy of the invoice was provided on this date. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department.     At the hand sink station at front line, soap dispenser lacked soap. Another soap in a bottle next to hand sink was available. [CA]: All hand sink stations shall provide soap in permanently installed soap dispensers. [COS]: PIC restocked soap dispenser.

    Missing testing method for Chlorine sanitizer. [CA]: Provide a testing method for Chlorine sanitizer (for ex: Chlorine test strip). Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds.

    1. Mop sink's maximum water temperature measured at 116F. [CA]: Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks. 2.
