- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
1. Facility is using fly sticky traps around the kitchen area at the time of inspection. [CA] Use approved methods to eliminate flies in the food preparation area. Recommend working with pest control to install more approved fly traps around the facility. Insect control devices that are used to electrocute or stun flying insects shall be designed to retain the insect within the device and be installed away from the storage and preparation of food or food contact surfaces and linens. 2. Caulking around the cook line is starting to fall apart. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin. Recommend having contractor remove old caulking and add fresh silicon.
- K33: Nonfood contact surfaces clean
Fan guards within the walk in have dust build up at the time of inspection. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
- K23: No rodents, insects, birds, or animals
Some flies noted within the kitchen area of the facility. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.

Two of the four light bulbs within the hood system area out at the time of inspection. [CA] Change out the light bulbs to illuminate under the hood system better.
- K45: Floor, walls, ceilings: built,maintained, clean
Some holes were noted around the electrical panel area in the back of the facility. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.
