Follow-up inspection; observed 2 live cockroaches and 14 dead cockroaches. Facility must remain closed until vermin have been completely eliminated and verified by the inspector. Facility is subject to a charged follow-up inspection and subject to overtime fees if requesting follow-up on weekend or after hours.

PIC stated the shrimp have been thawing for 1 hour and the ribs have been on the prep surface for 5 minutes. [COS] PIC put both PHFs back in the walk-in cooler to quickly cool.

Facility is using ROP for cooking process (sous vide). After the sous vide process, the packages are opened immediately and food is seared. No ROP storage at this time. Facility must provide SOPs to the Department by 10/15/2025 for the sous vide process.

    Observed potentially hazardous food that measured in the temperature danger zone (shredded cheese 49F). Employee stated he recently refilled the food prep unit. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Ensure PHFs being refilled into the food prep cold hold units are 41F or below. [COS] PIC put shredded cheese back into walk-in cooler

    Chlorine concentration measured at 25ppm in the sanitizer bucket. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium).

    Observed a few dead cockroaches in one of the monitoring traps. [CA] Eliminate vermin from facility; recommend professional/licensed pest control on a regular basis. Set monitoring traps Facility is expected to self-close to deal with the situation if any live vermin activity is observed.
