*REPEAT VIOLATION* California roll sushi measured at 44F and stored on the bottom outer edge of the open display case. Ambient temperature of display case on the outer edge measured at 44F. Per Staff, sushi roll was prepared approximately 2 hours prior to measurement, cooled down in the freezer, then relocated to unit approximately 30 minutes prior to measurement. Ambient temperature of unit on the inner side measured at 40F. [CA]: Potentially Hazardous Food shall be cold held at 41F or below at all times. [SA]: Sushi rolls were relocated to the inner part of the unit to cool down.
Bottom and outer edge of the open display case measured at 44F. [CA]: Cooling units shall be able to cold hold PHFs at 41F or below at all times. Assess/repair/adjust unit as needed. Discontinue storing PHFs on the bottom outer edge of the unit until it is able to properly cold hold PHFs.

Log observed. Discussed proper cooling methods with PIC. Ensure food items are properly cooled down to 41F prior to relocating to open display cooler.
One house fly observed in the facility. No direct contamination observed. [CA]: Remove fly using safe, approved and effective methods. Ensure food and food contact surfaces are protected at all times from possible contamination. Remove any standing water in facility.
Open display cooler's outer edge ambient temperature measured between 43F - 46F. [CA]: Repair/assess/monitor unit as needed. Ensure unit is able to cold hold Potentially Hazardous Food to 41F or below at all times.
