ROUTINE INSPECTION
- A follow-up inspection will be conducted to verify compliance of major violation. - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. * Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM).
One dead mouse observed on sticky trap on floor behind the one-door upright refrigerator. Rodent droppings observed in the following areas: - On floor underneath the three-compartment sink. - On floor behind the one-door upright freezer. - On top of two plastic storage bins placed below storage rack. - On top of one-door upright freezer. Per unit director, facility is serviced monthly. Last service was conducted on 2/6/2026. Pest control was provided for review. Per pest control report, evidence of rodents were observed with numerous recommended work orders. No contamination of food observed. [CA] Clean and sanitize area of old droppings. [COS] Monitoring trap with dead rodent was discarded. *** A follow-up inspection will be conducted. ** This Division recommends replacing milk crate containers used to store prepackaged snacks with plastic containers with tight fitting lids.
