OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Shanghai Bun Dynasty NEW OWNER: CBI Kitchen Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. A 25% penalty is accessed as facility did not apply for a new permit within 10 days after ownership change and start of operations. An invoice for the permit fee in the amount of $1,975.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 03/01/2025 - 02/28/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.     Employee eating and drinking in food preparation area. [CA] No employee shall eat or drink in the food prep, food contact storage, or food storage areas, use designated area. An open bottle of "Pod Juice" Jewel Mint with 5.5% NIC observed on shelf on top of food prep table at the back of walk-in fridge. [CA] No person shall store clothing or personal effects in any area used for the storage and preparation of food or food contact surfaces.

    Food handler card is not available to review at the time of inspection. PIC stated that employees were recently hired. Some employees were hired for 2 months.
