    Measured quaternary ammonium at sanitizer at 3 compartment sink at 0 ppm. Sanitizer bottle found empty. Employee replaced it with a new bottle, however sanitizer level still measured at 0 ppm. [CA] Maintained a minimum of 200 ppm quaternary ammonium. Repair the unit.

    Lack of chlorine test strips for dishwasher (white colored test paper). [CA] Provide and test at 50 ppm daily.

    Observed rodent droppings noted downstairs in the liquor storage room below the wire shelves. Observed 3-4 droppings on boxes of bar glasses. Observed several rodent traps set up below the shelving. No pest control report on site. Observed opening on ceiling and small opening behind wooden panel where pipes are. Facility shall cease and desist all food operations. [CA] Remove the droppings and clean and sanitize the area. Seal all holes on ceiling and where pipes are coming out. Provide pest control service. Pest control company serviced yesterday afternoon, reviewed report. - New traps have been set inside the liquor storage room and ceiling has been sealed to prevent entrance of vermin. - Floors have been cleaned and sanitized -Permit has been paid, receipt provided. Facility has been re-opened and ok to operate.

    Observed half open ceiling where metal pipe lines are located. [CA] Close up the ceiling to ensure facility is vermin proofed.

    Observed fresh rodent droppings downstairs in the liquor storage room. Observed old droppings in the bar in between the refrigerator and bar tap unit. Observed several rodent traps located throughout facility. Spoke with manager and they stated they are aware of issue and pest control has been coming out weekly. There is construction next door and that is where the rodents are coming from. [CA] Remove rodent droppings, clean and sanitize affected areas and contact pest control for service. Provide a copy of service report(s) for review at follow up inspection. Facility is closed due to rodent activity.
