--48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $ 1,816.25. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.

[CA] Ensure that all handwash sinks are able to provide warm water, soap, and single-use paper towels to allow employees to easily wash their hands when required. [COS] Employee refilled both dispensers.

Cover is missing from light fixtures located above food storage areas. [CA] [CA] Light bulbs shall be shielded, coated or otherwise shatter-resistant in areas where there is unpackaged foods, clean equipment, utensils, linens, or unwrapped single use articles.

Facility is operating without a valid permit as permit has expired on 3/31/26. [CA] A food facility shall not operate without a valid Health Permit. A 48 hour notice is issued. See comments section below.

One dead American cockroach observed inside the floor sink located near the mop sink. [CA] Food facility shall be kept free of vermin and vermin debris. Did not observed additional vermin activity elsewhere in the facility.
- K48: Plan review
Waffle makers have been added without approval prior submittal and approval from this department's plan check. Facility also does not have a ventilation hood for cooking equipment. [CA] The addition of cooking equipment shall be submitted to this department's plan check for review and approval prior to installation and use. Due to lack of ventilation hood, facility will also need a hood exemption from an engineer as part of the plan check submittal. Operator states that they will discontinue use of the waffle maker and remove it from the facility.
