    One dead cockroach observed on floor at the corner of the to-go/delivery driver pick-up corridor. Manager stated that facility is serviced monthly. Pest control reports were provided upon request and review. Per pest control reports; activity was not observed. [CA] Clean and sanitize area of dead cockroaches.

FOLLOW-UP INSPECTION

Follow-up inspection for closure due to cockroach activity. During time of inspection, observed facility is free of cockroach activity. No live activity or evidence of continuing harborage by the bar and kitchen areas. - Noted an area underneath the right end of the dishwashing area where is wooden board is installed. Noted accumulation and possible cockroach droppings in the back corner underneath the area. Clean and sanitize the area, and remove the wooden board or provide finishes to the area to prevent possible harborage. - Noted pest control report from 11/27/2024 stating activity was found in the bar area and cookline, and has been treated. Per operator, next control service to occur on Sunday, 12/01/2024. Cracks and crevices: - In the bar area, observed cracks and crevices underneath the bar counter, between the bar counter and the FRP panels. - In the kitchen area, observed cracks and crevices between wall mounted equipment including handwash stations. - In the kitchen area, at the cookline, observed a metal flashing installed over quarry tile cove base which appears to have a gap underneath. - Seal all cracks and crevices to prevent further harborage of vermin. Provide monitoring traps around potential areas of harborage.

ROUTINE INSPECTION

- On-site for a complaint investigation. - At time of investigation, imminent health hazard was observed. A subsequent limited inspection was conducted. - Facility is hereby closed due to evidence of a cockroach infestation. - See violation above. - Facility is to remain closed until infestation has been abated: - Clean and sanitize all affected areas. - Remove all live and dead cockroaches from facility. - If additional licensed pest control service is conducted, provide pest control report upon follow-up inspection. - Follow-up shall be billed at $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - Payment must be made prior to scheduling for follow-up inspection. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. - A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.

The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.

    Container of queso and queso blanco maintained inside the two-compartment hot holding unit at the cook line measured between 100F to 110F. One container of baked potato soup and one container of chili maintained at the front server station hot holding measured between 120F to 130F. Manager stated the items were reheated around opening time, less than one hour prior. [CA] RTE food from commercially processed, hermetically sealed container may be reheated to >135°F for hot holding. [COS] Sauces and soups were reheated to be hot held.
