Observed one dying cockroach by the handwash sink. Observed two dead cockroaches. Observed what appeared to be old/dried out rodent feces in one corner. Reviewed pest control report. Pest control services received the previous evening. [CA] Maintain facility free of vermin. Ensure that any dead cockroaches or old droppings are cleaned up and the areas sanitized.
FOLLOW-UP INSPECTION
This is a charged follow up inspection requested by owner. Observed two dead cockroaches, one in corner by water heater and one in the corner by the walk in fridge. Both were cleaned up and area was sanitized. No other evidence of vermin observed. Reviewed pest control report from treatment this morning and signed checklist. Observed all gaps and deficiencies appear to be sealed/fixed. Facility ok to open.
FOLLOW-UP INSPECTION
1. Observed Vermin: Documented in the following areas: Observed one live adult and two live juvenile cockroaches by hand wash/preparation sink. Observed on live juvenile cockroach on the wall by the front entrance. Observed one dead cockroach on lower shelf of preparation table, on the floor to the door by the dry storage. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: S. Lew 4. Notification: The person in charge during inspection, John, has been informed that the facility must remain closed immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

On site for catering evaluation. Observed major violation prior to starting catering evaluation.

Observed several live cockroaches behind the FRP next to the dry storage. Observed many deficiencies in structure including missing tiles on coving, gaps/holes in FRP. 2. ROUTINE INSPECTION
Observed gaps in cove base, holes in walls. [CA] Repair/replace cove base/flooring and walls to prevent the harborage of vermin.
FOLLOW-UP INSPECTION
Scheduled inspection with manager of facility. Facility has been treated and thoroughly cleaned. Observed three dead cockroaches. No other evidence of vermin found. Most of facility has had holes/cracks repaired and work will continue to eliminate harborage points. Review pest control report. Another treatment will be performed in two weeks and then monthly service after.
ROUTINE INSPECTION

Attempted inspection for caterer, observed multiple live cockroaches in varying stages of life around the hand wash sink, paper towel dispenser, food preparation sink, three compartment sink, dry storage area and under the stairs. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
