    1. Damaged floor tile noted inside of the mop sink. [CA] Replace broken floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable. 2. Hole noted on the ceiling were the soda lines go through. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. 3. Floor tiles are missing under the cashier station. Per PIC, they made repairs for new cashier stations installed and are waiting for a contractor. [CA] Replace missing floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable.

    Push cart in the back of the facility is damaged and has some pieces falling apart. [CA] Equipment and utensils shall be designed and constructed to be durable and retain characteristic qualities under normal use conditions.

    Door lock within one of the restrooms is damaged at the time of inspection. Per PIC, they have submitted a work order. [CA] Have door lock repaired. [SA] Facility has an additional restroom that is being kept in service. A "out of order" sign was posted in the restroom that is not operational.
