FOLLOW-UP INSPECTION

Follow-up for facility that was closed due to cockroach infestation. Upon inspection: - Pest control service and treatment was performed. Most recent pest control service report dated 3/24/26 was reviewed and notes no live activity. Monitoring glue traps were observed empty at time of inspection. - Two dead cockroaches observed under shelves by the kitchen entrance. Operator removed dead cockroaches and affected area was cleaned and sanitized. No signs of activity observed elsewhere in the facility. - Verified that the additional required items on reopening checklist has been completed. Facility has cleaned and sanitized all affected areas. Openings, gaps, or crevices that may harbor vermin has been sealed. Facility will continue to receive regular pest control services to prevent infestation from reoccurring with next service scheduled in two weeks. Facility is okay to resume operations. This is a subsequent follow-up inspection and facility be will billed $282.00

On site for 2nd follow up inspection. Facility was closed due to cockroach infestation on March 13, 2026. First follow up inspection was conducted on March 16, 2026. Note: This is a charged follow up inspection. Facility provided vermin checklist and pest control report from 3/19/2026. Live and dead cockroach activity observed in the facility. The facility shall continue to remain closed until all corrective actions on the provided checklist are completed. Continue to cease and desist all operations. Requirements Before Reopening: 1. Submit the provided re-opening checklist. 2. Obtain a copy of the latest pest control report from a licensed provider. Upon review, district specialist will schedule the follow-up inspection. - Clean and sanitize the affected areas and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed;. - Eliminate food and water sources inside and outside the facility. - Properly store items at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas) NOTE: A period of 24 hours from this inspection shall pass before requesting a follow-up inspection. Pest control treatments may require a sufficient amount of contact time to be effective. A follow-up inspection to verify compliance of major violations will be conducted upon facility request. The first follow-up inspection is free of charge.
