Follow up inspection conducted along with Supervisor Suzanne Lew and Larry Little. Facility was closed on 4/22/25 due to rodent infestation. On site met with Brandon Talbott, Store Team Leader and his team. Facility has cleaned rodent droppings noted during last inspection on 4/22/25. All food items except some frozen food items and alcohol section are removed from the premises to allow access to clean under sales aisles, produce islands, deli, bakery and back of the house area. No observations of any rodent activity in the facility during inspection. Per store team leader all contaminated surfaces have been cleaned and sanitized. Facility has installed Xcluder door sweeps at all the entry points around the facility to prevent vermin entry. Trees that were touching the building around the exterior have been trimmed to prevent vermin entry. Gaps around pipes under women's restroom hand wash sinks and in men's restroom have been fixed. All other structural issues noted by pest control company have been corrected. Pest control reports were provided by the facility to district specialist and supervisor weekly via email from March 2025 to present. Per comments on plan check final inspection conducted on 9/9/25, facility has corrected the following items: 1. Facility has removed existing handwash sink located near juice bar. 2. Prep counters and prep cabinets that are not easily moveable throughout the facility have been sealed to eliminate gaps. 3. Display refrigerator near meat area has been removed with a single door refrigerator on castors. Photos were taken during the inspection. Facility is ok to reopen.

ROUTINE INSPECTION

On site for complaint investigations. A limited inspection was conducted. Observed snap traps throughout the premises. Manager provided pest control reports for 4/20, 4/21 and 4/22. The pest control reports states that additional pest control measures have been requested by the facility due to increased activity. The increased activity throughout the facility indicates the infestation has expanded beyond the operations of the deli, juice bar and bakery into the premises of the facility. Manager provided pest control reports for 4/20, 4/21 and 4/22. The facility is hereby closed due to an active rodent infestation. CEASE AND DESIST ALL OPERATIONS.     1. In the produce walk in cooler observed a pooling of liquid under produce. 2. In the produce prep area observed a hole in the wall under one of the drain boards. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.

    1. Observed one of the coolers on the sales floor to be in disrepair. [CA] Ensure to maintain all equipment clean and in good repair. 2. In the seafood department, in the back of house area, observed cardboard being used to line shelves. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

    In the seafood department one of the hand sinks is missing hot water. [CA] Handwashing facilities shall be equipped to provide minimum 100°F water under pressure for a minimum of 15 seconds. Note: Per PIC, they wish to use the sink to rinse seafood items. Food preparation such as rinsing shall be conducted at the two compartment food prep sink. Ensure to maintain all hand wash sinks in good repair and fully stocked.

    In the public restroom the paper towel dispenser was empty. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Staff arrived to refill the dispenser.
