FOLLOW-UP INSPECTION

Follow up inspection conducted along with Supervisor Suzanne Lew and Larry Little. Facility was closed on 4/7/25 due to rodent infestation. On site met with Brandon Talbott, Store Team Leader and his team. Facility has cleaned rodent droppings noted during last inspection on 4/7/25. All food items except some frozen food items and alcohol section are removed from the premises to allow access to clean under sales aisles, produce islands, deli, bakery and back of the house area. No observations of any rodent activity or cockroach activity in the facility during inspection. Per store team leader all contaminated surfaces have been cleaned and sanitized. Facility has installed Xcluder door sweeps at all the entry points around the facility to prevent vermin entry. Trees that were touching the building around the exterior have been trimmed to prevent vermin entry. Gaps around pipes under women's restroom hand wash sinks and in men's restroom wall have been fixed. All other structural issues noted by pest control company have been corrected. Pest control reports were provided by the facility to district specialist and supervisor weekly via email from March 2025 to present. Photos were taken during the inspection. Facility is ok to reopen.

    Observed fresh rodent droppings in back kitchen area along the walls and under equipment, observed fresh droppings in the front service area. Observed fresh droppings in the walk in fridge and along the back corridor. Per PIC, they have pest control service daily, pest control reports reviewed. Observed the start of installation for wire mesh to close off areas above the walk in fridges. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.

FOLLOW-UP INSPECTION

This is the first follow up inspection to verify compliance with major violation K23. Observations: K23: Observed the back hallway area, the dosa wrap area, and the general facility to be cleaned of rodent droppings. Per PIC, the pest control came last night and they had a cleaning service to sanitize afterwards. Per PIC, they finished cleaning at 5 am and the store opened at normal hours (8:00 am). Per the service report provided, Western Exterminator company came on site on 3/26/2025 and provided service from 8:22:25 am - 10:26:25 am. Pest control service report stated an adult female roof rat was caught. Facility is okay to repoen.

    1. Major Violation: In the deli area observed several rodent droppings in the preparation area and several snap traps behind and under equipment. In the back of house area in the shared hallway with the entire facility observed numerous amounts of rodent droppings by swing doors separating the hallway to the main salesfloor. Also observed numerous rodent droppings in the area under the stairs to the office that was storing cleaning equipment.In the unused area across the deli observed numerous amounts of rodent droppings both old and fresh. 2. Minor Violation: In the bakery area observed several live german cockroaches of all life stages near the dishwasher and under the three compartment sink on the other side of the wall. Observed two dead german cockroaches on a wall behind a wire rack holding dishes. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
