This was a follow-up to the 6/9/25 and 6/18/25 inspections to assess compliance for vermin evidence/activity. Facility had completed and submitted the Checklist for Reopening after Closure due to Vermin Infestation a second time today, 6/19/25. Ecolab report provided from this morning around 7am stated no pest activity was found. Specialist inspected warehouse and kitchen; no evidence of rodents/droppings/cockroaches were observed in the facility. Observed facility had installed a large piece of plywood against the swing doors separating the warehouse from the kitchen. The swing doors were also lowered so that they were flush to the ground. Continue to work closely with pest control to monitor vermin activity. Operators stated Ecolab will continue to service the facility weekly for a month, then assess from there and continue working with Ecolab. Facility was re-opened. Consulted supervisor S. Lew.

FOLLOW-UP INSPECTION

This was a follow-up to the 6/9/25 inspection. Facility was closed due to evidence of active vermin activity in the warehouse. Provided checklist for reopening after closure. When the checklist has been completed, call (408) 918-3400 to schedule a reopening. Follow-up inspections will each be billed per hour. Consulted supervisor M. Huato and director B. Santiago.

    In the warehouse: observed approximately 15-20 rodent droppings on the floor against the walls, between pallets/shelving. Observed one deceased large cockroach. The warehouse opens into the kitchen. There are swinging doors dividing the two areas, but the doors are not able to be fully closed/sealed. Observed openings in the door as part of the door construction. PIC stated that since the 6/9/2025 inspection, no pest control service has been administered. PIC stated the warehouse was cleaned on Friday, 6/13/25. [CA] A food facility shall not operate when there is a vermin infestation or has evidence of that is not eliminated or has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s) that cannot be corrected. The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. Obtain pest control service, seal all openings/cracks/crevices, and sanitize all surfaces. FACILITY WAS CLOSED DUE TO EVIDENCE OF ACTIVE RODENT ACTIVITY. ***See notes below.***

Notes: - PIC stated the facility only uses the 3-compartment sink to manually wash dishes, and they don't use the dishwasher. Before using the dishwasher again, ensure it is able to sanitize dishes in 50ppm chlorine. - A follow-up inspection will be conducted. The first follow-up is free of charge. Any subsequent inspections will be billed $298/hr during business hours. Due to technical difficulties, this report will be emailed.

    In the warehouse area: observed approximately 5 large deceased cockroaches in sticky traps. Observed approximately 5-8 old rodent droppings on the floors, along the walls/under shelving. No other evidence of vermin was observed. [CA] Keep facility clear of vermin and droppings. Note: monthly pest control reports were provided from ecolab. Some reports mentioned some rodent activity. A follow-up inspection will be conducted to confirm compliance.

  • K30: Food storage: food storage containers identified

    Observed open bags of food in the dry storage room. [CA] After opening bags of food, store inside rigid containers with tight-fitting lids.

  • K41: Plumbing approved, installed, in good repair; proper backflow devices

    Observed the ice machine's drain pipe extended into the floor sink. Observed 2-compartment sink faucet was leaking. [CA] Maintain a minimum one inch air gap between the drain pipe and the floor sink rim to prevent wastewater backflow. Repair the leak at the 2-compartment sink.

    Measured hot water at 3-compartment sink and prep sink to be approximately 108F. [CA] Provide hot water (120F) at these sinks.

ROUTINE INSPECTION

Notes: - PIC observed empty paper towel dispenser in the center prep area during the inspection, and paper towel dispenser was immediately refilled. Ensure the paper towel dispenser is stocked at all times. - PIC stated the dishwasher is currently not in use and all dishes are washed manually until the busy season/school year starts. PIC was unable to turn on the dishwasher during the inspection. Ensure dishwasher is able to sanitize dishes at 50ppm chlorine before use.
