ROUTINE INSPECTION
Notes: - Per Bijan, the property owner will be erecting an enclosed patio to match the patio at the adjacent facility. Following the patio cover, they will expand their menu to include wine. Ensure to submit to our plan check unit for any structural changes to the kitchen or front service area (sandwich and cafe area). - Per Rocio and Bijan, they have pest control services bimonthly. Most recent pest control report was for December 2025. Service report indicated no pest activity. - The food safety certificate expires in one week. Ensure to maintain a valid food safety certificate. - A follow up inspection will be conducted to verify compliance with K08 and K23.
Timers were not on for the croissants and steamed milks. Per staff, they had been made at 7 am. [CA] PHFs utilizing time as a public health control (TPHC) shall be discarded within 4 hours from the point in time when food is removed from temperature control. [COS] Items were discarded per TPHC guidelines. Educated on importance of complying with TPHC written procedures.
Dead cockroaches were observed in the following areas: - smushed against a cabinet door in the front service area. - dead on a sticky trap under the stove - dead under an upright cooler. [CA] Clean and sanitize area of dead cockroaches. Provide pest control reports for the next 2 months.
1. Repeat Violation: There is a ceiling tile missing above the dry storage area that holds the flours. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. 2. The corner guard near the stove is detaching. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair. Reattach and seal.
1. The paper towel dispenser in the kitchen was empty. A roll of paper towels was available nearby. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. 2. Repeat Violation: The hand sink in the back prep area was obstructed. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times.
The toilet is leaking. [CA] Ensure to maintain toilet and toilet facilities in good repair.
1. The faucet at the front of house handwash station is leaking. 2. The water line at the kitchen handwash station is leaking. [CA] Repair plumbing fixture and maintain in clean and good repair.
The stove and sheet trays next to the stove have an accumulation of debris and grease. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Repeat Violation: Bulk food items in the dry storage area were not stored in containers with lids. [CA] Store open bulk foods in approved NSF containers with tight fitting lids.
FOLLOW-UP INSPECTION
This is the first follow up for the routine inspection where the facility was closed for cockroaches. Observations: K22: The mop sink is draining properly. K23: Facility was serviced by Terminix on 4/24/25. One live cockroach was observed moving slowly and dying. PIC eliminated the cockroach. No other evidence of cockroaches observed. Facility is okay to reopen. Continue to correct any and all pending violations.
ROUTINE INSPECTION

1. Scoop handles in the flour bins were in direct contact with the flours. [CA] Store scoop handle in such a way that the handle does not come in direct contact with food to prevent contamination. 2. Facility is reusing sour cream and yogurt containers to hold jams and jellies. [CA] Single use food container (e.g. soy sauce container, milk carton, apple juice container, can food, wine bottle) shall not be reused to store food. 3. Repeat Violation: Pressurized tanks of carbon dioxide are not secured to a rigid structure. [CA] All pressurized cylinders shall be securely fastened to a rigid structure.
1. Major Violation: Live cockroaches of all life stages were observed in the front area of the facility near the hand sink and the juice presser. A dead cockroach and hatched ootheca were observed in the back of house near the small storage area under the stairs. Cockroach droppings were found near the small storage area under the stairs. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [CA] Ensure to maintain the facility free of animals, pests, and vermin.

The mop sink is not draining. [CA] Food facility shall not operate if there is sewage overflowing or backing up in the food facility. The food facility, or impacted areas, shall remain closed until all plumbing problems have been corrected and all contaminated surfaces have been cleaned and sanitized. [SA] There is a sanitary sewer available in the back premises of the building. The mop sink is elevated and located out of the way from food preparation areas. Correct within 24 hours.
