OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Mykonos Grill Inc. NEW OWNER: Mykonos Grill Inc., Eligio Ramirez-Quinonez The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $1,580.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 05/01/2025 - 04/30/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 04/22/2025. *Permit condition: none. *Ensure all employees have the Food Handler's Card.
Observed large dent on the seam of the can of grape leaves stuff with rice. Can was not observed bulging. [Corrective Action] Ensure canned foods are safe, wholesome and in good condition. [Corrective on Site] Operator discarded the canned grape leaves stuffed in rice.
Observed pink mold like substance in the ice chute of the soda fountain. [Corrective Action] Thoroughly clean and sanitize.
ROUTINE INSPECTION
Observed plastic/rubber portion of the spatula broken and showing signs of deterioration. [Corrective Action] Replace the spatula to prevent physical contamination of foods.
Lack of Food Safety Certificate. [Corrective Action] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times.
Observed a knife stored in the crevice between the wall and the work table. [Corrective Action] Discontinue storing the knife in the crevice.
Handwash station at the back kitchen observed partially blocked by rolling cart and dirty towels hamper. [Corrective Action] Ensure handwash sink is easily accessible at all times. Re-locate rolling cart and dirty towels hamper.
FOLLOW-UP INSPECTION
A follow-up inspection was conducted to verify violations from routine inspection conducted on 08/13/2024 are in compliance. K06: Handwash sink at the back kitchen observed accessible. K07: Food items in the walk-in cooler measured below 41 degrees Fahrenheit. K40: Chlorine concentration in the sanitizing bucket measured at 100 ppm. K41: The drain pipe for the prep sink observed re-attached. Continue to work on violations noted on the last inspection report.
