- On-site for follow-up inspection to verify compliance of K23m - observation of rodents, insects, birds, or animals. - Facility has conducted the following: - All shelving in the warehouse has been modified to provide permanent shelving off of the floor. - Pallets previously used to stored food off of the floor has been removed for ease of cleaning and monitoring of traps. - Additional shelving are being installed to reduce the use of pallets in the facility. - Previously inoperable roll-up door has been repaired to separate the main warehouse from the staging/dumpster area. - Owners will be discussing with plaza landlord regarding the removal of vegetation on the side of the warehouse. - Affected areas have been cleaned and sanitized of rodent droppings. Any and all contaminated food was discarded per supervisor. - Per supervisor, frequency of service has been increased to twice weekly until vermin has been abated.

[COS] Water from tank was immediately drained. All shellstock was maintained on ice.

* Note: A follow-up inspection will be conducted to verify compliance of K23. - Upon follow-up, any additional signs of rodents will result in suspension of health permit. - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Clean and sanitize the affected area(s). - Dispose of any and all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal).

    Refuse area observed with accumulation of garbage, food debris, and standing water. [CA] Refuse enclosure not kept clean of debris to prevent buildup of soil and attractants for vermin. Refuse containers shall be collected or cleaned frequently enough to minimize the development of objectionable odors or conditions that attract or harbor insects or rodents.

    Heavy accumulation of debris observed on floors underneath shelving in the warehouse. [CA] Regularly clean under equipment to prevent accumulation of debris.

    Rodent dropping observed on floors underneath pallets and shelving throughout the warehouse. Observed no rodent droppings on sales floor or contamination of food or food contact surfaces. Supervisor stated that facility is serviced weekly by pest control company. [CA] Clean and sanitize areas of old droppings. Live flies observed in seafood department, meat department, and in the warehouse. [CA] Use any and all approved methods to abate live flies.
