FOLLOW-UP INSPECTION

A follow up is requested today following a closure for cockroaches during routine inspection. PIC provided the last pest control service request, dated 10/20/2025, indicating no vermin. No evidence or signs of cockroach activity observed during this inspection. No oysters on site during this inspection. Ensure to maintain oyster tag with original container until no more oysters in container, keep tag for 90 days in an organized manner. Observed hand sinks stocked. Observed chlorine test strips available for use. Per PIC, technician has been called to service bar dish machine. Manually sanitize using 100 ppm chlorine. No PHFs hot holding observed during this inspection; ensure to maintain PHFs at 135F or higher. - Per PIC, all areas have been cleaned and sanitized. Continue with professional pest control service to monitor and maintain facility free of vermin. Please be advised that there is a follow up fee of $298, minimum one hour during business hours. FACILITY IS APPROVED TO REOPEN.

Follow up inspection requested following closure for cockroaches on 10/20/2025. Specialist observed evidence of dead cockroaches in the following locations: - on right side of hood - inside the upright 3 door reach in - under the dry food storage shelving - under the prep sink - fecal droppings observed on dishes stored inside the 3 door upright reach in 2. Photographs: Taken for documentation purposes: yes 3. Supervisor Notified: Yes 4. Notification: The person in charge during inspection, Elmer Orellana , has been informed that the facility must remain close. Facility is to remain closed and thoroughly clean and sanitize all impacted areas. Facility shall remain closed until written approval to reopen in obtained from this Dept. Ensure facility is cleaned and sanitized and there is no evidence of dead or live vermin (rodent or cockroach) during the follow up or facility will remain closed for imminent health risk.
