Observed boxes stored directly on floor in small storage room. [CA] Properly store boxes in a clean manner, 6 inches above the floor and away from the walls to prevent pest harborage.
FOLLOW-UP INSPECTION
On site for follow up inspection after limited inspection on 4/24/25 and an owner initiated follow up inspection on 4/26/25. Observed pest control reports from 4/28/25 and 4/30/25. - pest control report from 4/28 mentions two adolescent rodents were caught in the back warehouse. - pest control report from 4/30 mentions no rodent activity observed in the facility. At the time of the inspection, no rodent activity observed throughout the sales floor or at the back of the house storage. Floors under the food and non food sales aisles are cleaned. All the pallets or racks with storage in the back warehouse have been moved away from the walls to allow cleaning and room to inspect for rodent activity. Observed snap traps and glue traps placed under the food aisles and in warehouse in multiple locations. Continue to obtain pest control services as needed and maintain facility free of any vermin activity.
Observed a small gap in ceiling panel in small side storage room with exit door. [CA] Replace the ceiling panel to remove hole in the structure and maintain in good repair to prevent pest harborage.
FOLLOW-UP INSPECTION
Note: 1.This is a follow-up inspection to the limited inspection which was conducted on 4/24/2025. The store was closed due to the presence of dead rodent beneath the muffins aisle in aisle 3. Abundant rat droppings were also observed on some aisles over the stored. During today's follow-up inspection, no live or dead rodent was observed. The base covers for most of the aisles were removed and the floor area is cleaned. However, trash and rodent droppings were observed in some aisles which were overlooked by the store manager. The store is made to remove the base covers of the aisles which have not removed the parts and clean the floor area. The store is open for service now. However, the manager is told to conduct another through cleaning on the floor areas, top of the some of the storage shelved and fridges, improve the storage condition back in the receiving area, and seal some of the holes found on the wall in the receiving area. Another follow-up shall be conducted on Monday by a district inspector. Suzanne Lew, Supervising Registered Environmental Health Specialist is consulted about the situation. 2. Since this is an owner initiated after hour follow-up inspection, our office bill the owner $645.00 follow-up inspection fee. Any upcoming follow-up inspections shall be conducted at a charge of appropriate fees as well.
Boxes of merchandises were stored directly on the floor or on wood pallets near the wall back in the storage area which does not provide access for cleaning. [CA] For easy floor cleaning and to prevent insect/rodent infestation and hiding, store all utensils/equipment at least six inches off the floor and away from the wall on approved shelf/dunnage rack at all the time
Heavy dirt, slime, and spill accumulations were observed on the display and storage shelves of some of the aisles. [CA] Conduct a thorough cleaning on the aisles immediately.
There are some holes on the wall back in the storage area. There area some damaged ceiling panels in the front area as well. [CA] To prevent insect/rodent access and hiding, fix the structural deficiencies observed back in the store.
Numerous rodent droppings were observed under the seasonal aisles which have not been cleaned. [CA] The premises of each food facility shall be kept free of vermin. Please remove the base covers of the aisles and remove the droppings and trashes accumulated immediately.
ROUTINE INSPECTION
CO0156695 Limited inspection conducted along with complaint investigation. Manager Gilmer is aware of the vermin issue. He stated on Thursday 4/17/25, a rat came from the back side door when it was open for 2 hours for delivery. Pest control report from ECOLAB dated 4/22/25 was provided. The report mentions to seal open gaps and holes or add weather stripping. The report also mentions to replace missing sealing tiles above aisle 3 with food products. Per manager, ECOLAB also came in on 4/23/25 to place traps under the aisles and around the facility. CEASE AND DESIST ALL OPERATIONS Facility is hereby closed due to active rodent infestation. Facility shall comply with all the action items listed on the pest control report. Clean and sanitize all areas with rodent droppings and dead rodent. Discard any food that is contaminated or adulterated due to vermin. Provide a report from pest control service indicating all the action items are corrected and facility has taken necessary measures to prevent vermin entry. Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55.
Observed dead rodent on a sticky trap on floor under the muffins aisle in aisle 3. The floor under the aisle is accessed by lifting the bottom shelf. Observed numerous rodent droppings under the chips and peanuts aisles. The bottom of the aisle is accessed by lifting the bottom shelf. Observed open food packets of chips and peanuts and peanuts under the aisle on floor in aisle 4 on snacks side. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved.
