- K33: Nonfood contact surfaces clean
1. Large amount of food waste/debris (assorted meat refuse) on the floor in the walk-in refrigerator. [CA] Nonfood contact surfaces shall be kept clean and maintained. 2. Buildup of dust/debris observed on fan guards in the walk-in. [CA] Fan guards shall be kept clean, particularly in the presence of uncovered meat.

Raw meat observed to be stored directly over vegetables in walk-in refrigerator. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

Observed dead cockroaches in the following locations: - 1 dead roach on top of residential freezer unit in the deli area - 2 dead roaches under the 2 door reach in freezer in the deli area - 2 dead roaches on top of unused water cooler - 5+ dead roaches in a trap under the prep sink - 10+ dead roaches of varying life stages (adult, juvenile) behind freezer unit reach-ins, 7 of which are in a sticky trap and 3 of which are on the back of the unit itself [CA] Clean and sanitize area of dead cockroaches or old droppings. The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. [COS] Employees removed dead cockroaches. Last pest control treatment was conducted on 6/21/25, and pest control report was reviewed. A follow-up inspection will be conducted within a week to ensure that the facility is free of vermin.

Observed large square hole in the ceiling above the shelving unit adjacent to the restroom. [CA] Seal hole to prevent harborage of vermin.
- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
Multiple pieces of unused equipment observed within the deli area of the facility. [CA] Equipment that is not in use shall not be stored on the premises in order to prevent the harborage of vermin.
- K30: Food storage: food storage containers identified
Potatoes observed to be stored on the floor in mesh bag in the grocery aisle. [CA] Food shall be stored a minimum of 6 inches off the floor.
- K35: Equipment, utensils: Approved, in good repair, adequate capacity
Stained cardboard is being used to line shelves in the grocery area. [CA] Discontinue the use of cardboard to line shelves, and line shelves with material that is smooth, nonabsorbent, and easily cleanable to prevent the attraction of vermin.

Facility could not provide a valid FSC. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times.
