Facility conducts preparation and packaging of rice cakes to be distributed for wholesale. Facility has posted license for Processed Food Registration from California Department of Food and Agriculture as of 2018. Facility shall obtain an updated Processed Food Registration.

Observed multiple large bulk bags of rice are open and uncovered. [CA] After opening, bulk foods shall be transferred to an approved container with tight fitting lids.
- K34: Warewash facilities: installed/maintained; test strips
Facility lacks testing materials for sanitizer of choice. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

**REPEAT VIOLATION** Observed the handwash sink in the kitchen is fully obstructed by large standing equipment. Per PIC, employees utilize the 3-compartment sink for handwashing. Nearby handwash sink is available for use. [CA] Handwash sink shall be fully accessible and unobstructed at all times.

Observed all refrigeration units lack thermometers. [CA] A thermometer +/- 2°F shall be provided and correctly located in each hot and cold holding unit containing potentially hazardous foods.

1) Observed holes in the grease trap underneath the 3-compartment sink. [CA] Seal all gaps in the facility. The facility shall be fully enclosed to prevent pest entrance and harborage. 2) Observed multiple large pieces of unused equipment and clutter in the facility, especially nearby the back door. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.

** REPEAT VIOLATION** 1. Facility lacks active food manager safety certificate. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. 2. Actively working employee lacks food handler card. [CA] All food handlers shall maintain a valid food handler card for the duration of their employment.
- K14: Food contact surfaces clean, sanitized
Observed lack of sink stopper available for use to fill up the 3-compartment sink for warewashing and sanitization purposes. No active warewashing at the time of inspection. [CA] Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. [SA] Specialist directed employee to set up the sanitization compartment with plastic during the inspection.
