Foods are received from Madhuban Events and Catering within the same India Community Center facility. No Potentially Hazardous Foods where observed in the refrigeration units. Change the name of correspondence from Raj Desai to Som Sharma.

Paper towels were not properly dispensed from the paper towel dispenser. Loose roll was observed on the side. [Corrective Action] Ensure paper towels are properly dispensed from the paper towel dispenser. [Corrected on Site] Technician repaired the paper towel dispenser.

1. Ambient air temperature in the single door upright cooler was measured at 70 degrees Fahrenheit. [Corrective Action] Ensure refrigeration unit is able to maintain cold holding temperature of 41 degrees Fahrenheit or below. 2. The ambient air in the cooler of the cooler/freezer combo was measured at 46 degrees Fahrenheit.
