Food manger certification has expired. [CA] Obtain a new food manger certification within 60-days.

Dust build up noted on the fan guards of the walk in at the time of inspection. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Sprinkler system within walk in freezer is leaking at the time of inspection. [CA] Repair plumbing fixture and maintain in clean and good repair.

Cardboard is being used to line shelves within the walk in and flooring by the cashier station. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

Frozen chicken was thawing inside of prep sink on its own. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

All PHFs in under counter refrigeration was measured between 77-80*F at the time of inspection. Per PIC, a contractor is coming to look at unit. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] All food within refrigeration unit was VCD at the time of inspection. VCD form was provided.
