[COS] PIC voluntarily discarded the PHFs in the food prep cold hold unit.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: MR FALAFEL LLC NEW OWNER: MR FALAFEL INC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/1/2024 - 11/30/2025 This report serves as a temporary permit. Use rapid cooling techniques such as frequent stirring, shallow pans, ice paddles, ice as an ingredient, ice baths and/or blast chiller. [COS] PIC uncovered rice containers and moved the eggplant to reach-in refrigerator to complete the cooling process.
