1. Container of pork cheeks stored near the handwash sink below the paper towel dispenser in the kitchen. [Corrective Action] Do not store any foods near the handwash sink to prevent contamination. 2. Containers of sauce observed stored on the ice in the ice machine. [Corrective Action] Do not store anything on the consumable ice.

Dead cockroaches observed on the monitoring traps near the water heater. [Corrective Action] Replace monitoring traps. Clean and sanitize areas of dead cockroaches.

REPEAT VIOLATION. Lack of paper towels in the paper towel dispenser at the handwash sink in the kitchen and service area. Loose paper towels were observed nearby. [Corrective Action] Maintain paper towels inside the paper towel dispenser.

Food items in the infrared warmer were not monitored by log or written time. Per PIC, the fried fish was cooked and placed on the warmer an hour ago. [Corrective Action] Ensure Potentially Hazardous Foods not maintained at proper temperature and properly maintained using written discard time or logged.
- K43: Toilet facilities: properly constructed, supplied, cleaned
Toilet paper observed stored outside of the toilet paper dispenser. [Corrective Action] Store the toilet paper inside the toilet paper dispenser.
