[Corrective Action] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. 2. Lack of employee Food Handler's Card. [Corrective Action] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

    Faucet is not able to fully shut off at the food prep sink. [Corrective Action] Repair faucet to stop the leak.

    Grime accumulation on the drain pipe of the food prep sink. [Corrective Action] Thoroughly clean and maintain clean.

    1. MAJOR. Lack of any paper towels at the handwash sink in the restroom. [Corrective Action] Ensure paper towels are always available to properly dry off hands. [Suitable Alternative] Loose napkins were placed near the handwash sink in the restroom. 2. Lack of paper towels in the paper towel dispenser at the handwash sinks at the service area and back area. Loose paper towels were available nearby. [Corrective Action] Maintain paper towel dispenser stocked at all times.

    1. Lack of chlorine test strips. [Corrective Action] Provide chlorine test strips to be able to monitor chlorine concentration. 2. Bleach is lemon scented. [Corrective Action] Use unscented bleach to sanitize utensils.

FOLLOW-UP INSPECTION

On site for follow up inspection for routine inspection conducted on 12/4/24. No observations of dead or live cockroaches in the facility. The areas of dead cockroaches from last inspection has been cleaned. Pest control reports were emailed to inspector by PIC. Repair loose coving in the walkway near freezer and door to the back prep area. Continue to obtain professional pest control services as needed.

    Observed toilet paper is stored outside the dispenser. [CA] Toilet paper shall be stocked in dispenser to prevent contamination.

    Observed brown mold-like substance on the interior panel of the ice machine. [CA] Clean and sanitize ice machine. Food contact surfaces shall be kept clean as required.

    In the walk-in cooler, observed accumulation of dust on the fan. [CA] Non-food contact surfaces shall be kept free of accumulation of dust, dirt, food residue, and other debris.

    * Repeat Violation. Observed several dead cockroaches in juvenile stages in the following locations: - In the front, 1x underneath the counter and by the floor sink - In the side area by restrooms, 2x inside the ice chest - In the kitchen, 1x underneath the 3-compartment sink and 1x underneath the desk No live activity was observed during the inspection. [CA] Eliminate cockroaches and clean and sanitize affected areas. Premises shall be kept free of vermin. Observed pest control invoice from October. Per PIC, pest control visited more recently. PIC will provide the updated report.

  • K36: Equipment, utensils, linens: Proper storage and use

    Observed knives stored behind the handwashing sink and below the paper towel dispenser. [CA] All cleaned utensils shall be stored in a clean, dry location where they are not exposed to splash.
