- On-site for follow-up inspection for facility evaluation for ownership change. - Facility has corrected the following violations: - K23M - Observed rodents, insects, birds, or animals: - Facility was treated prior on 3/26/2026, on 3/31/2026, and again on 04/07/2026. Pest control reports were provided for review. - Latest service indicated no activity was observed by technician. - Facility has deep cleaned and sanitized the entire facility. - All walls in facility has been cleaned and repaired of all holes, gaps, and crevices. - Ceiling panels have been cleaned and sealed of all gaps. - Equipments have been deep cleaned and sanitized. - At time of follow-up inspection, no evidence of vermin was observed. _________________________________ OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Famtime Thai Street Food NEW OWNER: Wun Suk Corporation The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $2,281.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 05/01/2026 - 04/30/2027 This report serves as a temporary permit.
